Class One and Two Combined Form, Class Two requires a physician's
signature, this form when signed by a physician is good for 3 years. |

Class Three form required for long term campouts for all adults 40
and older, this form must be signed by a physician and must be renewed
yearly. |

Application Cards (Print on Blue Card Stock) |
You can add your name, merit badge counselor, merit badge name and
other info to this form before you print it on cardstock, print one
page, put it back in the printer and print the second page on the back |

Information the troop needs to make you a Merit Badge Counselor, you
